Service: Health Wellness

Our mission is to enhance community health by providing the individual and family lifestyle habit interventions and ongoing support needed to produce better health to last a lifetime. Our main focus is to fill the gap that exists between primary care physician recommendations and the patient actually attempting to follow those recommendations. The majority of patients simply do not know where to begin when faced with the task of developing better lifestyle habits when diagnosed with, or at risk for, chronic health conditions. Our organization works in partnership with primary care providers and pediatricians to provide the education, guidance, and ongoing support that individuals and families need for the restoration and maintenance of optimum health.
